Telephonic RN Case Manager – Special Needs Plan, Bilingual Preferred

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver telephonic case management services to members of the Special Needs Plan who are medically complex and chronically ill.

• Carry out thorough health assessments and develop personalized care plans.

• Facilitate care coordination with both internal and external partners, including doctors and specialists.

• Provide education to members and caregivers regarding disease management and preventive healthcare.

• Track member progress and advocate for timely and appropriate interventions.

• Identify and assist in resolving any service or access issues that may affect the quality of care.

• Conduct all communications via phone, email, and Teams.

• Execute essential physical tasks such as talking, hearing, standing, walking, sitting, handling objects or controls, reaching, and occasionally lifting or moving items up to 10 pounds.

⛳️ Requirements

• Possession of an active, unrestricted RN license in California (Non-Compact).

• A minimum of 2 years of clinical nursing experience is required.

• At least 1 year of experience in case management is necessary.

• Proficiency in Microsoft Office applications (Word, Excel, Outlook) is essential.

• Willingness to obtain RN licensure in Nevada, Arizona, North Carolina, and Texas following hire.

• Bilingual candidates are preferred, particularly those who speak Spanish, Korean, Mandarin, or other languages.

• Prior experience with health plans or IPAs is preferred.

• A Bachelor's Degree in Nursing (BSN) is preferred.

• Must maintain a HIPAA-compliant workspace.

• Availability to work during Pacific Time hours is required.

• This position is not open to LVNs.
