Bilingual Patient Coordinator, English – Spanish

This is a fully remote position, open to applicants in Colombia.

📋 Description

• Organize work assignments based on the training and experience of Home Care Workers

• Engage in communication with patients, their families, Home Health Aides (HHAs), clinicians, contractors, and various departments

• Precisely document interactions and pertinent information in HHA Exchange

• Oversee and address open cases to ensure prompt resolutions

• Assess the needs of patients and their families, coordinating suitable care

• Inform HHAs about changes in service days/hours, discharges, and other relevant updates

• Provide information to healthcare professionals and other relevant stakeholders

• Prepare daily, weekly, and monthly reports

• Handle requests for overtime and travel time

• Utilize Smart Map and other resources to locate aides, provide directions, and verify availability

• Complete and document incident reports as necessary

• Manage mutual and linked cases effectively

• Monitor and respond to emails, voicemails, and faxes

• Adhere to agency guidelines, policies, and procedures

• Adapt to changes within the agency and the healthcare sector

• Forward concerns or inquiries to the appropriate supervisor

• Carry out additional duties as assigned to meet agency requirements

⛳️ Requirements

• Proficiency in both English and Spanish — mandatory

• Prior experience with HHA Exchange — essential

• Technologically adept and comfortable utilizing healthcare software and digital tools

• Strong written and verbal communication abilities

• Exceptional customer service and active listening skills

• Strong organizational and time-management capabilities

• Ability to multitask, prioritize, and manage several cases at once

• Capacity to work efficiently under pressure in a fast-paced environment

• Positive and team-oriented mindset

• Flexible and adaptable to changing situations

• Keen attention to detail and accurate documentation

• Must be available for weekend on-call support when necessary
