This is a fully remote position, open to applicants in Florida.
• Act as the main relationship manager for a portfolio consisting of 20–25 CareStation partners.
• Assist partners during the onboarding process and integrate the CareStation into their comprehensive community health strategy.
• Set shared objectives and agreed-upon goals at the commencement of each partnership.
• Ensure clarity regarding roles and responsibilities, holding both OnMed and partners accountable.
• Monitor and evaluate partner-level outcomes data.
• Conduct forward-looking, mission-driven partner reviews.
• Bring partner insights back to OnMed to inform the product and deployment roadmap.
• Identify opportunities for partners to enhance their engagement and aid in building the internal case.
• Document community outcomes across the portfolio to contribute to OnMed's broader impact narrative.
• Undertake additional related responsibilities as assigned.
• Report to the Partner Impact Director.
• A bachelor's degree is required; a degree in Public Health, Public Policy, Social Work, or a related field is preferred.
• Over 5 years of experience in a client-facing or partner-facing role within community health outreach, nonprofit or public-health consulting, or social-impact strategy.
• Experience in management consulting, nonprofit strategy, or impact investing with a client-facing element.
• Professional experience working with underserved communities.
• Native-level proficiency in Spanish, both spoken and written.
• Fluent Spanish with professional-level verbal and written communication skills.
• Capability to influence without direct authority.
• Experience in assisting organizations to translate mission into measurable results.
• Comfort in navigating complex, multi-stakeholder environments such as health systems, government agencies, and community organizations.
• Excellent written and verbal communication skills suitable for both C-suite and community health worker audiences.
• A sincere personal commitment to health equity and community access.
• Direct experience in community health, FQHCs, public health programs, or initiatives related to social determinants of health is advantageous.
• Ability to maintain high standards for partnerships while strengthening relationships.
• Proficiency in utilizing data and analytics tools to monitor partner health, identify trends, and support recommendations with data.
• Willingness to travel occasionally, up to 30–40% of the time.
