This is a fully remote position, open to applicants in Canada.
• Provide assistance to patients and healthcare professionals concerning patient support services, diseases, and therapies.
• Aid patients and caregivers with evaluations related to patient support services.
• Serve as a liaison between patients, families, and healthcare providers regarding access to medications.
• Address inquiries related to programs, disease states, and products using the provided materials.
• Support patient adherence and direct clinical queries outside of program materials to healthcare providers.
• Record patient information and interactions in electronic patient records.
• Conduct regular file reviews and updates.
• Adhere to case-management protocols, including data gathering, patient follow-up, monitoring therapy disruptions/discontinuations, reporting adverse events, and coordinating with primary healthcare providers.
• Collaborate with reimbursement specialists and physicians to secure funding for therapies and explore alternative options.
• Organize and monitor patient services in alignment with KPIs to facilitate therapy access.
• Engage in specialized therapy training sessions.
• Participate in pharmaceutical meetings, patient meetings, and conferences; collaborate with patient associations and advocacy groups.
• Coordinate with healthcare providers, pharmacies, and infusion/injection clinics for treatment logistics and resolution of issues.
• Maintain communication with the manager, team, and client regarding patient cases.
• Assist in training new Nurse Case Managers.
• Provide feedback on protocols, databases, processes, and interdepartmental workflows.
• Report adverse events and serious adverse events in accordance with program procedures.
• Perform additional assigned duties and participate in standby/on-call coverage as necessary.
• Bachelor’s degree or licensed nursing diploma (IA), higher education degree, or specific office management experience is preferred.
• Five (5) years of experience in nursing.
• Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment is mandatory.
• At least two (2) to five (5) years in healthcare, pharmaceutical, insurance, or related fields.
• Experience in public or private third-party reimbursement, sales, care management, clinical support in the pharmaceutical industry, or similar experience.
• Fluency in French and/or English may be necessary depending on the program and geographic location.
• Experience in hematology and/or nephrology is advantageous.
• Strong skills in patient-centered care, multitasking, pressure management, customer service, communication, and teamwork.
• Proficient in internet research and computer skills.
• Proficient in Microsoft Word, Excel, PowerPoint, and other Office applications.
• Ability to communicate effectively and comprehend medical information.
• Willingness to travel at least 15% of the time each year.
