Bilingual (English/Spanish) Transitions of Care Behavioral Health Social Work Case Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver short-term interventions to members referred to community-based behavioral health services due to an immediate need or post-hospitalization.

• Help prevent unnecessary emergency room visits and/or hospital readmissions.

• Conduct thorough psychosocial assessments via phone or video with members and their caregivers.

• Implement clinical social work interventions, which include brief therapeutic techniques, support with family dynamics, assessments of social determinants of health, and safety evaluations.

• Work collaboratively with clinicians, nurses, and care coordinators to provide integrated, patient-focused care for patients with high needs.

• Identify and escalate complex cases to the BH Medical Director for further consultation.

• Engage in team huddles, panel reviews, interdisciplinary team rounds, and contribute to program and role development.

• Record all clinical interactions in the electronic health record.

⛳️ Requirements

• Master’s degree in Social Work with a current license in any US state - LCSW, LMSW, or equivalent state clinical license.

• Openness to obtain additional state licenses.

• At least 5 years of experience in behavioral health or care management.

• Bilingual proficiency in English and Spanish.

• Skilled in conducting comprehensive psychosocial assessments via video and phone.

• Capable of independently managing a fully remote work schedule from Monday to Friday, 8:30 am to 5:00 pm CST or PST.

• Experience in Geriatric Behavioral Health or the health insurance sector is preferred; familiarity with Medicare Advantage is a bonus.
