This is a fully remote position, open to applicants in California.
• Oversee, schedule, and adjust clinical appointments within the care team.
• Handle referrals to specialists and external clinical services.
• Follow up with families to minimize care gaps and reduce no-show rates.
• Facilitate care transitions and ensure continuity among the interdisciplinary team.
• Provide clinical updates to physicians who have made referrals.
• Coordinate lab work and the necessary documentation with referring providers.
• Assist providers for patients who require escalated care.
• Deliver evidence-based education to families about managing chronic conditions.
• Explain medication regimens, treatment plans, and care objectives in easy-to-understand language.
• Address inquiries from patients and families through messaging and phone calls.
• Promote health literacy and empower families to take an active role in their child's care.
• Initiate and oversee medication prior authorization requests with insurance companies.
• Collaborate with pharmacies to resolve dispensing problems and coordinate refills.
• Monitor authorization statuses and escalate any delays as needed.
• Maintain precise medication records and document communications with pharmacies.
• Record clinical interactions across various EHR and care management systems.
• Engage in interdisciplinary case conferences, team huddles, and quality improvement sessions.
• Report on patient progress and care gaps to clinical supervisors.
• Identify workflow inefficiencies and contribute to solutions for process improvements.
• Current California Registered Nurse (RN) license in good standing.
• Associate's, Bachelor's, or Master's degree in Nursing is acceptable.
• Proven proficiency with various technology platforms and electronic health record (EHR) systems.
• Excellent organizational, communication, and interpersonal abilities.
• Capability to manage competing priorities and collaborate effectively in a remote, virtual environment.
• Comfort working with diverse pediatric populations that are medically and socially complex.
• Enthusiasm for joining a rapidly growing startup and refining new care models.
• Reliable internet connection and a dedicated workspace appropriate for remote work.
• Bilingual in English and Spanish; must possess medical proficiency in Spanish.
• Minimum of 1–2 years experience as a Nurse Care Coordinator.
• Availability to work full time Monday through Friday, 9 AM to 6 PM PT.
• Experience in an outpatient primary care environment.
• Preferred: experience in pediatric, chronic care, or care coordination settings.
• Preferred: knowledge of prior authorization processes and payer requirements.
• Preferred: experience in underserved or urban settings.
