This is a fully remote position, open to applicants in Canada.
• Offer assistance to patients and healthcare professionals by providing information about Patient Support services, illnesses, and treatments.
• Aid patients and caregivers with evaluations related to Patient Support services.
• Serve as a liaison between patients, families, and healthcare providers regarding medication accessibility.
• Respond to inquiries concerning program services, disease conditions, and products.
• Deliver continuous support to enhance patient adherence.
• Refer patients to healthcare providers for any clinical inquiries that extend beyond program materials.
• Record patient information and interactions within electronic patient records.
• Carry out ongoing file reviews and updates throughout the patient’s journey.
• Adhere to case management protocols, including data gathering, follow-up, monitoring therapy disruptions or discontinuations, reporting adverse events, and coordinating with primary healthcare providers.
• Work collaboratively with reimbursement specialists and physicians to obtain therapy funding and explore alternatives.
• Organize and monitor patient services in alignment with KPIs to streamline therapy access.
• Engage in specialized therapy training and participate in pharmaceutical meetings.
• Attend patient meetings and conferences, and collaborate with patient associations and advocacy groups as necessary.
• Coordinate with healthcare providers, pharmacies, and infusion/injection clinics to manage treatment logistics and resolve issues.
• Maintain communication with the manager, team, and client regarding patient cases.
• Assist in training new Nurse Case Managers.
• Report adverse events and serious adverse events in accordance with program protocols.
• Perform additional duties as assigned and participate in standby/on-call coverage when necessary.
• A Diploma in healthcare or a related discipline; a bachelor’s degree is preferred.
• Active registration as a Licensed Nurse (RN or RPN/LPN) from the College of Nurses in the province of employment is mandatory.
• A minimum of two (2) to five (5) years of experience in healthcare, pharmaceuticals, insurance, or related fields.
• Preferred background in healthcare and case management within medical/hospital settings, insurance, or the bio-pharmaceutical industry.
• Proficiency in French and/or English may be necessary based on the program and geographic location.
• Experience with patient support, case management, medication access, reimbursement, adverse event reporting, and electronic patient records.
• Capability to work independently as well as collaboratively within a team.
• Strong analytical and problem-solving abilities.
• Competence in working effectively across functions and within a small team.
• Ability to consistently produce high-quality professional work in a fast-paced environment.
• Experience in training new Nurse Case Managers and providing insights on protocols, databases, processes, and interdepartmental workflows.
• Availability for standby/on-call duties may be required.
