Bilingual Spanish Telephonic RN Nurse Case Manager – Special Needs Plan, California RN Required

This is a fully remote position, open to applicants in California, +1 more state.

📋 Description

• Deliver telephonic case management services to Special Needs Plan members who are medically complex and chronically ill.

• Perform thorough health assessments and develop tailored care plans for each member.

• Collaborate with both internal and external partners, including physicians and specialists, to coordinate care effectively.

• Provide education to members and their caregivers regarding disease management and preventive healthcare practices.

• Track member progress and advocate for timely and appropriate interventions.

• Identify and assist in resolving any service or access issues that may affect the quality of care.

• Manage all communications through phone, email, and Teams.

• Work remotely from home while adhering to Pacific Time hours.

⛳️ Requirements

• An active and unrestricted RN license in California (Non-Compact) is mandatory.

• A minimum of 2 years of clinical nursing experience is required.

• At least 1 year of experience in case management is necessary.

• Proficiency in Microsoft Office applications (Word, Excel, Outlook) is needed.

• Willingness to obtain RN licensure in Nevada, Arizona, North Carolina, and Texas is essential.

• A HIPAA-compliant workspace must be maintained.

• Must be available to work during Pacific Time hours.

• Bilingual candidates, particularly in Spanish and Vietnamese, are highly preferred.

• Previous experience with health plans or Independent Practice Associations (IPA) is advantageous.

• A Bachelor's Degree in Nursing (BSN) is preferable.

• This position is not open to Licensed Vocational Nurses (LVNs).
