Bilingual Spanish Telephonic RN Nurse Case Manager – Special Needs Plan

This is a fully remote position, open to applicants in Arizona, +5 more states.

📋 Description

• Deliver telephonic case management for members with medical complexities and chronic illnesses.

• Perform thorough health assessments and develop personalized care plans.

• Collaborate with both internal and external partners, including healthcare providers and specialists, to coordinate care.

• Provide education to members and caregivers regarding disease management and preventive health measures.

• Track member progress and advocate for timely and suitable interventions.

• Identify and assist in addressing service or access challenges that affect the quality of care.

• Conduct all communications through phone, email, and Teams.

• Adhere to Pacific Time hours, working Monday to Friday from 8:00 AM to 5:00 PM PT.

⛳️ Requirements

• Valid, unrestricted RN license in California (Non-Compact).

• A minimum of 2 years of clinical nursing experience is required.

• At least 1 year of experience in case management.

• Proficient in Microsoft Office Suite (Word, Excel, Outlook).

• Willingness to obtain RN licensure in Nevada, Arizona, North Carolina, and Texas is necessary.

• Bilingual candidates are highly preferred, especially in Spanish and Vietnamese.

• Prior experience with a health plan or Independent Practice Association (IPA) is a plus.

• A Bachelor's Degree in Nursing (BSN) is considered advantageous.

• A HIPAA-compliant workspace is required.

• This position is not available to LVNs; a California RN license is mandatory.
