Bilingual – Spanish Speaking Health Coach

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform outbound telephonic outreach to patients and offer personalized coaching and support for those managing chronic conditions.

• Assist patients in implementing tailored care plans and achieving their objectives through education and coaching that emphasizes behavior modification, nutrition, physical activity, and self-management.

• Utilize motivational interviewing techniques and establish SMART goals.

• Carry out thorough assessments of patients' health conditions, lifestyle choices, dietary habits, and readiness for change.

• Provide education to patients regarding disease management, adherence to medication, symptom identification, and preventive strategies.

• Execute care delivery tasks in accordance with Cadence clinical policies and established procedures under suitable clinical oversight.

• Track patient progress, adherence to treatment plans, and health outcomes through regular check-ins and remote monitoring tools.

• Record patient interactions, outcomes, and follow-up plans within the Cadence platform.

• Identify obstacles to care, including social needs, and connect patients with resources or internal support pathways.

• Collaborate in real-time with clinicians and patient support teams using the Cadence Platform, Google Workspace, and Slack.

• Provide feedback to enhance workflows.

⛳️ Requirements

• A valid NBHWC health coaching certification is mandatory.

• A minimum of 1 year of experience as a health coach is essential.

• Experience in delivering patient education and support to individuals managing chronic conditions is required.

• Readiness for remote work, including a reliable high-speed WiFi connection, is necessary.

• Proficiency in Spanish is required.

• Familiarity with AI-assisted clinical tools, such as Gemini, Open Evidence, or similar platforms is needed.

• Capability to communicate effectively with providers and patients regarding the role of AI in care delivery and the irreplaceable nature of human judgment.

• Ability to adhere to the designated Monday through Friday schedule within United States time zones.
