This is a fully remote position, open to applicants in United States.
• Process, monitor, and manage patient referrals to guarantee timely appointment scheduling.
• Arrange appointments for primary care physicians (PCP), dental, vision, specialists, and transportation for patients.
• Confirm patient insurance eligibility and verify the patient’s in-network PCP.
• Handle Release of Information (ROI) and Authorized Representative (AR) requests.
• Ensure accurate documentation of communications and referral activities within internal systems.
• Collaborate effectively across teams with care team members to promote continuity of care and deliver exceptional patient experiences.
• Participate in ongoing improvements to referral processes and scheduling efficiency.
• Act as the foundational scheduling support for Pair Team’s care coordination framework.
• Work closely with Case Managers, healthcare providers, and health plans to facilitate seamless transitions between services, ensuring clear communication and adherence to HIPAA regulations.
• Proficiency in both English and Spanish (written and spoken).
• At least 1 year of experience in healthcare operations, referral coordination, or appointment scheduling in a dynamic healthcare setting.
• Previous roles as a Referral Coordinator, Scheduling Coordinator, or in a healthcare administrative capacity are highly preferred.
• Demonstrated ability to handle high-volume scheduling and referral processes while maintaining precision and attention to detail.
• Strong organizational abilities with experience managing multiple systems, calendars, and priorities concurrently.
• Comfort with navigating insurance portals, verifying eligibility, and confirming provider/network details to ensure timely scheduling.
• Exceptional written and verbal communication skills, capable of interacting professionally and empathetically with patients, providers, and care teams.
• Familiarity with technology such as Google Suite, Slack, and data tools (e.g., Looker or similar platforms).
• A reliable, HIPAA-compliant home workspace with a stable internet connection.
• A dedication to enhancing healthcare access for underserved populations.
• Must reside in the United States.
• Must be available to work during PST hours.
• Participation in E-Verify to confirm employment eligibility for new employees.
