Bilingual Home Care Coordinator – Spanish, English

This is a fully remote position, open to applicants in Colombia.

📋 Description

• Act as a point of contact among contracts, patients, their families, home health aides (HHAs), clinicians, aides, and various departments.

• Facilitate efficient communication and identify issues during attendance verifications, introductory calls, and other engagements.

• Assist in the provision of high-quality care.

• Fulfill necessary tasks in accordance with agency protocols and policies.

• Organize work assignments based on the training and experience of Home Care Workers.

• Reduce the number of open cases to guarantee prompt resolutions.

• Accurately document communications in the HHA Exchange.

• Relay issues or modifications to patients, family members, contracts, clinicians, aides, and other departments.

• Supply essential information to healthcare professionals.

• Collaborate with family members and support personnel to ensure continuous care and identify client requirements.

• Inform HHAs about discharges and alterations in service days or hours.

• Prepare daily, weekly, and monthly reports.

• Oversee overtime and travel time requests.

• Utilize Smart Map and other resources to find aides, provide directions, and check their availability.

• Fill out incident reports.

• Manage mutual and linked cases.

• Promptly open and respond to emails, voicemails, and faxes.

• Adjust to changes within the agency or industry.

• Discuss concerns and inquiries with the supervisor.
