Youth Care Coordinator – PT, PRN

This is a fully remote position, open to applicants in Maryland.

📋 Description

• Collaborate with youth aged 6–22 and their caregivers to assess needs and set goals.

• Deliver in-person case management services tailored to the authorized level of care.

• Conduct a minimum of two face-to-face visits each month in various settings such as homes, schools, or community locations.

• Engage weekly on behalf of clients and families through activities like research, resource coordination, collateral contacts, system navigation, advocacy, follow-up calls, and documentation.

• Evaluate needs and link clients and families with relevant resources, services, and support systems.

• Assist clients and families in navigating the healthcare and education systems while helping them understand their options.

• Connect participants to appropriate programs and services based on assessed needs.

• Promote Youth Care Coordination through outreach initiatives targeting schools, pediatrician offices, and other referral sources.

• Timely completion of required documentation and progress notes.

• Engage in bi-weekly supervision sessions.

⛳️ Requirements

• A Bachelor's degree in human services or a related field is REQUIRED, along with 1 year of experience in mental health, including mental health peer support.

• A Bachelor's degree in a non-mental health field and 2 years of verifiable mental health experience, which includes mental health peer support.

• Possess reliable transportation and a valid driver's license.

• Strong organizational skills and ability to follow through on tasks.

• Capability to work independently while being accountable for responsibilities.

• Experience with or a sincere interest in working with youth and families.

• Proficient in using technology and electronic medical records systems.
