Virtual Nurse Practitioner

This is a fully remote position, open to applicants in California.

📋 Description

• Conduct virtual wellness visits for adults and/or pediatrics.

• Perform health assessments and other telehealth services.

• Review the caseload and prepare necessary technology and tools for patient services.

• Manage the daily schedule utilizing company technology.

• Collaborate with clinical in-home team members (Sprinters) who visit patients at their homes.

• Establish friendly connections with a diverse range of patients.

• Gather pertinent patient information and provide various services.

• Conduct health risk assessments, medication evaluations, and cognitive screenings.

• Offer patients educational resources regarding health and wellness.

• Work alongside Sprinters to assess vital signs and coordinate blood draws.

• Execute tasks tailored to meet patients’ specific needs.

• Provide care coordination and management.

• Deliver preventive care interventions and oversee medication management.

• Refer patients to specialists and community resources as needed.

• Document all findings accurately.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner License in California.

• Credentialed Medicaid provider for California (Medi-Cal).

• Previous experience in conducting wellness visits.

• Demonstrate professionalism, integrity, accountability, confidentiality, care, and compassion.

• Openness to perform last-minute problem-solving and think creatively.

• Technologically adept and comfortable using laptops or mobile devices for documentation.

• Proficient in using HIPAA-secure messaging applications for care coordination.

• Excellent written and verbal communication skills.

• Ability to work autonomously or collaboratively within a team environment.

• Must successfully pass a national background check and valid clinical license verification.
