Per Diem RN, Triage

This is a fully remote position, open to applicants in Illinois, +8 more states.

📋 Description

• Address overnight and weekend inquiries related to changing needs of hospice patients.

• Provide answers and collect information from patients and their caregivers.

• Evaluate patient requirements and manage/direct hospice services and care via phone conference.

• Guide patients and caregivers on suitable nursing interventions.

• Assess intervention alternatives and apply critical thinking to choose the appropriate care.

• Identify the necessity for nursing visits using the EMR and information from calls.

• Assign visit RNs or LPNs as necessary.

• Complete, submit, and uphold precise clinical documentation.

• Document changes and outcomes in pain and symptom management.

• Relay patient needs and condition updates to physicians and secure orders.

• Encourage patient self-sufficiency and comfort care.

• Collaborate and provide guidance to families, caregivers, and the hospice interdisciplinary team.

• Ensure that equipment and requisite services are organized.

• Uphold patient confidentiality and appropriately handle protected health information.

• Engage in quality assessment and performance improvement initiatives.

• Execute other assigned responsibilities.

⛳️ Requirements

• Valid Registered Nurse (RN) license in the state(s) of practice.

• Must possess or acquire an RN license for the 10 states served and/or a compact license prior to hire.

• Graduate from an accredited professional school of nursing.

• Currently licensed, certified, or registered in the state(s) of practice.

• Capable of passing DHS background checks, consumer reports, and drug screenings.

• If driving, must have a valid state driver's license, a well-maintained automobile, required automobile insurance, and an acceptable driving record.

• Preferred 1+ years of recent experience in hospice or home care.

• Preferred 2+ years of nursing experience.

• Ability to push/pull and lift/carry up to 30 pounds, occasionally up to 50 pounds.

• Flexibility to adapt to various work conditions in a home environment.

• Adherence to HIPAA regulations and standards.
