Virtual Clinician, Home Evaluations, Nurse Practitioner / Physician Assistant

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct online Video Visits lasting approximately 30 minutes with members enrolled in Medicare Advantage, Medicaid, and selected commercial plans.

• Review the medications and medical histories of members.

• Educate members on their health and respond to their inquiries.

• Assist in closing gaps in care and facilitate connections to suitable healthcare services.

• Conduct Video Visits in a private environment with a reliable internet connection.

• Participate in regular meetings with management, team members, and clinical staff.

• Achieve operational, clinical, and member experience key performance indicators and metrics.

• Maintain necessary training, affiliations, credentials, and compliance standards.

• Interact with health plan members remotely using a computer, phone, iPad, and various other electronic devices.

⛳️ Requirements

• Full-time, board-certified nurse practitioner or physician assistant.

• Active state licenses in the relevant coverage areas.

• Maintain board certification.

• FNP or AGNP certification is preferred for Nurse Practitioners.

• Ability to obtain and maintain multi-state licenses in the coverage area as needed.

• Preferred experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth delivery, or a comparable format.

• Trained and credentialed for all health plan clients and lines of business in primary coverage states.

• Availability for 40 hours each week.

• Consistently deliver 320+ capacity units per month.

• Maintain a pace of 220+ completed visits monthly.

• Adhere to training, affiliation, and compliance requirements for Video Visits.

• Possess broadband connectivity with enabled video and audio.

• Ensure a quiet environment with a neutral background for visits.

• Ability to protect health plan member privacy according to CVS Health guidelines.

• Strong verbal, written, and electronic communication skills.

• Capacity to work independently and practice autonomously.

• Proficient in information management and technology.
