This is a fully remote position, open to applicants in Michigan.
β’ Transform HarmonyCares' clinical strategy into a unified and scalable home-based care model across various markets.
β’ Develop the framework for the home-based care model, which includes team roles, visit frequency, care planning, handoffs, escalation procedures, and long-term accountability.
β’ Convert evidence into actionable pathways addressing frailty, serious illness, transitions, medication risks, behavioral health, and caregiver requirements.
β’ Establish enterprise-wide standards along with suitable local adaptations.
β’ Create routines for high-risk reviews, panel management, interdisciplinary care, after-hours responses, and closed-loop follow-ups.
β’ Oversee the clinical design of population health programs and assess feasibility and impact prior to scaling.
β’ Collaborate with Quality and field medical leaders to enhance Stars and HEDIS performance, ensure the integrity of risk adjustment documentation, and address safety and utilization drivers.
β’ Define interdisciplinary practice expectations, competencies, onboarding procedures, clinical education, and professional support.
β’ Work together on workforce planning, maximizing clinician capabilities, clinician retention, and performance enhancement.
β’ Utilize case reviews, metrics, and field feedback to identify variations and alleviate workflow burdens.
β’ Co-create clinical workflows and decision support systems with Product and Technology, engaging front-line clinicians in the testing process.
β’ Establish expectations for the adoption and benefits of clinical tools, including automation and AI.
β’ Mandate patient safety measures, human oversight, and criteria for modifying or discontinuing ineffective tools.
β’ Execute other job-related tasks as assigned.
β’ Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) with an active, unrestricted license.
β’ Current board certification in Family Medicine, Internal Medicine, or Geriatric Medicine through a member board of the American Board of Medical Specialties (ABMS) or the American Osteopathic Association (AOA).
β’ Over 10 years of senior leadership experience in developing and operating interdisciplinary care models within a large, distributed delivery organization.
β’ Previous experience translating evidence and strategy into practical workflows, roles, tools, training, and measurable results.
β’ Proven expertise in clinical operations, quality improvement, workforce practices, change management, and implementation.
β’ Strong comprehension of complex Medicare populations and the economics of value-based care.
β’ Established credibility with both physicians and non-physician clinicians, along with a successful track record of leadership across professional boundaries.
β’ Preferred experience in home-based care, complex primary care, geriatrics, palliative care, community health, or longitudinal care management.
β’ Formal training in quality improvement, implementation science, clinical informatics, public health, or healthcare administration is preferred.
β’ Previous experience with technology-enabled care delivery and clinical product development.
