Virtual Clinician – Home Evaluations, Nurse Practitioner or Physician Assistant

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct online Video Visits lasting approximately 30 minutes with members of Medicare Advantage, Medicaid, and select commercial plans.

• Review members' medications and medical histories.

• Educate members regarding their health and address any inquiries.

• Assist in closing care gaps.

• Facilitate connections to suitable care tailored to each member’s unique needs.

• Operate in a private location with a reliable internet connection.

• Engage in regular meetings with managers, teams, and clinical staff.

• Achieve operational, clinical, and member experience KPIs and metrics set by Signify Health.

• Provide only Video Visits/telehealth services.

• Adhere to training, affiliation, and compliance requirements.

⛳️ Requirements

• Must be a board-certified nurse practitioner or physician assistant.

• Possess active state licenses in the areas of coverage.

• Capability to obtain and sustain multi-state licenses in the coverage area, as necessary.

• FNP or AGNP certification is preferred for nurse practitioners.

• Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or similar formats is preferred.

• Availability of 40 hours per week is required.

• Consistently provide 320+ capacity units each month.

• Maintain a pace of 220+ completed visits per month.

• Comply with training, affiliation, and Video Visit requirements.

• Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s).

• Keep board certification current.

• Fulfill remote visit delivery requirements, including broadband connectivity with enabled video and audio.

• Ensure a quiet environment with a neutral background for conducting visits.

• Uphold health plan member privacy according to CVS Health guidelines.

• Exhibit effective verbal, written, and electronic communication abilities.

• Ability to work independently and practice autonomously.

• Proficient in information management and technology.

• Capability to interact remotely with members, including sitting, writing, operating a computer and phone, speaking clearly, and hearing member responses.

• Ability to view and read computer screens, iPads, and other electronic devices.
