Virtual Care Nurse Practitioner – PRN

This is a fully remote position, open to applicants in New York, +3 more states.

📋 Description

• Provide comprehensive, evidence-based care utilizing a remote technology platform.

• Evaluate, diagnose, treat, and manage various health concerns.

• Offer health counseling and develop disposition plans for patients aged 18 months and older.

• Educate patients about pregnancy prevention, STI prevention, and safer sexual practices.

• Assess patients in primary care, those with acute conditions, chronic illnesses, and in transitional care.

• Deliver counseling on contraceptive care and manage related medications.

• Instruct patients on health maintenance and address their care inquiries.

• Record all patient interactions in an EHR in accordance with MinuteClinic policies and procedures.

• Collaborate with internal and external colleagues for coordinated patient care, including the broader patient-centered medical home.

• Participate as a member of a patient care team that includes Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare professionals.

• Operate independently, prioritize tasks, solve problems, take initiative, and advocate for both patients and the practice.

⛳️ Requirements

• Valid license in one of the following states: CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV.

• At least two years of relevant medical experience or equivalent qualifications.

• Strong verbal, written, and electronic communication skills.

• Excellent organizational skills with the ability to manage multiple tasks.

• Initiative, problem-solving skills, adaptability, and flexibility.

• Capability to work independently and practice autonomously.

• Proficient in information management and technology.

• Ability to collaborate with professional colleagues as needed to ensure quality care.

• Proficiency in additional languages, such as Spanish, may be advantageous depending on the market.

• Completion of a Master's Degree level Family Nurse Practitioner program.

• Current National Board Certification.

• State license to practice in the Advanced Practice Nurse role where employed.

• Availability to provide at least 9 hours of service per week, including every third weekend.

• Access to reliable broadband and a quiet environment with a neutral background for remote consultations.

• Commitment to maintaining patient privacy in accordance with CVSH guidelines.

• Ability to operate a computer and phone, communicate clearly, hear patient responses, and view/read screens and electronic devices.
