Triage Registered Nurse, Hospice

This is a fully remote position, open to applicants in Mississippi.

📋 Description

• Conduct initial, comprehensive, and ongoing evaluations of patients and families to identify hospice requirements.

• Offer thorough physical evaluations and medical histories of current and past illnesses via telephone.

• Deliver professional nursing care utilizing all components of the nursing process.

• Assess and monitor patient conditions.

• Utilize health assessment information to establish nursing diagnoses.

• Create care plans with objectives based on nursing diagnoses and palliative nursing interventions.

• Involve patients and families in the planning phase.

• Initiate preventive and rehabilitative nursing actions.

• Administer medications and treatments according to the physician’s care plan.

• Provide guidance to patients and families regarding nursing and related requirements.

• Offer health care instructions based on assessments and plans.

• Assist patients with daily living activities, promoting self-sufficiency and comfort care.

• Serve as case manager and coordinate patient care for an assigned group of patients.

⛳️ Requirements

• Must possess a valid RN license in California.

• A minimum of two years of experience is required, with hospice experience being essential.

• Must have experience with Home Care Home Base.

• Exhibits excellent observational, problem-solving, and communication skills, both verbal and written; nursing abilities as per competency checklist.

• Demonstrates capability to organize and prioritize workload independently.

• Capable of lifting, positioning, and/or transferring patients.

• Able to lift supplies and equipment as necessary.

• Maintains current CPR certification.

• Self-motivated and capable of working with minimal supervision.
