Transitions of Care Behavioral Health Case Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver short-term support to members transitioning to community-based behavioral health services due to emerging needs or post-hospitalization.

• Assist in avoiding unnecessary emergency room visits and hospital readmissions.

• Conduct thorough psychosocial evaluations via phone or video with members and their caregivers.

• Execute clinical social work interventions, which include brief therapeutic sessions, guiding families through support challenges, assessing and addressing social determinants of health, and completing safety evaluations.

• Collaborate with clinicians, nurses, care coordinators, and fellow team members to provide integrated, patient-centered care for patients with high needs.

• Clinically assess and refer complex cases to the BH Medical Director for further consultation.

• Engage in team huddles, panel evaluations, interdisciplinary team discussions, and the growth and development of programs/roles.

• Record all clinical interactions in the electronic health record.

⛳️ Requirements

• Master’s degree in Social Work.

• Possession of an active clinical license in any US state — LCSW, LMSW, or a comparable state clinical license.

• Openness to acquiring additional state licenses.

• A minimum of 5 years of experience in the behavioral health or care management sectors.

• Proficient in performing comprehensive psychosocial assessments via video and phone.

• Capable of independently managing a fully remote work schedule, Monday to Friday, 8:30 AM to 5:00 PM PST or MST.

• Bilingual in English and Spanish, Creole, or Vietnamese is preferred, but not mandatory.

• Experience in Geriatric Behavioral Health or the health insurance industry (Medicare Advantage experience is highly valued).
