Transfer Nurse, LVN – PRN

This is a fully remote position, open to applicants in Texas.

📋 Description

• Deliver high-quality, timely support in triaging and coordinating transfers from other facilities.

• Monitor and address the triage requirements of the transfer center.

• Complete the direct referral acceptance procedure.

• Assess patients’ clinical requirements utilizing technology.

• Communicate quick acceptance or deflection outcomes.

• Collaborate with referral sources, physicians, nurses, and other involved parties.

• Report to and maintain accountability to the Director of Assessment and Referral for Voyages.

⛳️ Requirements

• Valid LVN license in the State of Texas.

• Prior experience in the behavioral health and substance use sector is preferred.

• Experience in case management, hospitals, and admissions is preferred.

• Exceptional customer service abilities.

• Strong verbal and written communication skills.

• Organizational and multitasking capabilities.

• Ability to work varied shifts in support of a 24/7 department, including day and overnight shifts.

• Shift hours may vary across different US time zones.
