Telephonic Nurse Care Manager

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Engage with a designated member population in Illinois via phone communication.

• Conduct health assessments, develop care plans, and provide education regarding chronic conditions and healthcare plan gaps.

• Utilize strategies, methods, and techniques to address members' physical, environmental, and psychosocial health challenges.

• Identify and eliminate obstacles to effective care.

• Continuously track patient care through assessments, data analysis, discussions with members, and active care planning.

• Ensure patients make progress toward their health goals.

• Navigate various systems while applying critical thinking skills.

• Deliver tailored assessments, ongoing care planning, and expert advice over the phone.

• Assist members in overcoming care barriers, managing chronic conditions, and accessing essential resources.

⛳️ Requirements

• Bachelor’s degree in nursing (BSN) or a related health field.

• Active Registered Nurse (RN) license in Illinois with no disciplinary actions or the ability to obtain IL RN licensure.

• At least 3 years of clinical acute care experience as a licensed RN.

• Background in health promotion, coaching, and wellness.

• Bilingual proficiency in English/Spanish, English/Polish, or English/Creole (preferred).

• Experience in case management, discharge planning, and patient education for adult acute care (preferred).

• Managed care experience (preferred).

• Certified Case Manager (CCM) credential (preferred).

• Previous experience with Medicare & Medicaid recipients (preferred).

• Familiarity with community health and social service agencies and additional community resources (preferred).

• Minimum internet download speed of 25 Mbps and upload speed of 10 Mbps.

• Suggested connection types include wireless, wired cable, or DSL.

• A dedicated workspace free from ongoing interruptions to safeguard member PHI/HIPAA information.
