Telephonic Nurse Care Manager

This is a fully remote position, open to applicants in Florida.

📋 Description

• Facilitate care coordination for patients with diverse healthcare requirements

• Evaluate patients’ healthcare needs and conduct telephonic assessments

• Examine patient data and clinical documentation

• Recognize potential obstacles to care and formulate suitable individualized care plans

• Educate patients and caregivers while addressing inquiries regarding care plans

• Collaborate with physicians, home health clinicians, hospitals, and various healthcare professionals

• Track patient progress and identify any changes in their condition

• Organize services and assist patients in accessing available healthcare resources

• Apply clinical judgment to identify potential issues and escalate them as necessary

• Accurately and promptly document patient interactions

• Uphold confidentiality and adhere to relevant policies and regulatory standards

⛳️ Requirements

• Current and unrestricted Registered Nurse license

• 3–5 years of experience in clinical acute care

• A minimum of 2 years of experience in home health

• Proficient in reviewing clinical information and utilizing nursing judgment to assess patient needs

• Familiarity with electronic medical records and other computer-based healthcare systems

• Prior experience in care management, case management, utilization management, or telephonic nursing is preferred

• Experience in coordinating care among hospitals, physicians, home health agencies, and other healthcare providers is preferred

• Background working with patients who have chronic or complex medical conditions is preferred

• Strong understanding of healthcare resources, care coordination, and transitions of care is preferred

• Experience in a remote or telephonic clinical setting is preferred
