This is a fully remote position, open to applicants in Tennessee.
• Manage the daily operations of the Clinical Care Coordination team.
• Conduct regular supervision and facilitate team meetings.
• Supervise referral triage, case conferencing, documentation evaluation, training adherence, and productivity oversight.
• Offer coaching, mentoring, and professional growth opportunities to team members.
• Track and assess care coordination performance metrics.
• Perform monthly audits and deliver timely internal reports to the Program Manager.
• Ensure accurate and prompt tracking of case statuses for assigned referrals.
• Assist in the recruitment, onboarding, and training of new clinical care coordinators and operational support staff.
• Provide clinical care coordination services, including managing caseload coverage when necessary.
• Address manager requests and support departmental processes, including handling phone lines and daily operational tasks.
• Aid in external provider consultation and coordination by reviewing and processing service continuations.
• Act as a point of contact for routine outreach inquiries concerning services and referrals.
• A Master’s degree in Social Work, Psychology, or a related field is required.
• A Bachelor’s degree with a minimum of 3 years of leadership experience may be acceptable.
• Supervisory experience is essential.
• Military affiliation is mandatory.
• Applicants must be current or former service members, spouses or family members of active-duty service members or veterans, or Gold Star spouses or family members.
• At least two years of experience providing behavioral health or social services within the military community is required.
• Proficiency in Microsoft Office 365 Applications, CRM Platforms, Zoom, basic Exploratory Data Analysis & Descriptive Statistics, and Adobe is necessary.
• Strong ability to communicate effectively through written, verbal, in-person, and virtual channels.
• Capability to establish and achieve simultaneous short- and long-term objectives.
