This is a fully remote position, open to applicants in Pennsylvania.
• Conduct administrative care management tasks, which include member outreach, responding to inbound calls, and arranging services.
• Engage in phone outreach to members concerning next steps in their care plans, available community or health plan resources, scheduling inquiries, concerns, and ongoing education.
• Link members to health plan and community resources.
• Act as the primary support for inquiries, requests, and concerns from both members and providers.
• Clarify procedures and protocols related to care plans.
• Assist with member onboarding and administrative responsibilities, such as dispatching welcome letters, correspondence, and educational materials.
• Forward requests and inquiries to management as necessary.
• Document and uphold non-clinical member records in compliance with state and regulatory mandates.
• Aid People Leaders with weekly data reporting, constituting up to 50% of the role.
• Share findings and data with People Leaders and colleagues.
• Contribute to the training of new employees.
• Adhere to all policies and standards.
• Perform additional duties as assigned.
• High School diploma or GED is required (minimum).
• 2–4 years of relevant experience is preferred.
• Over 4 years of experience in processing healthcare or insurance authorizations and coordinating services for members with access to Medicaid Long-Term Services and Supports (LTSS).
• Excellent problem-solving, communication, organizational, and time management abilities.
• Capacity to adjust to evolving business needs with openness, flexibility, and professionalism.
• Must be adept at presenting findings clearly and concisely to a variety of stakeholders.
• Competent Excel skills, including data extraction, pivot table creation, and conditional formatting, or a willingness to enhance skills using Gen AI tools.
• Availability to work Monday–Friday, from 8:00 AM to 5:00 PM (EST).
