Social Worker I

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee participants of AltaMed Managed Care assigned to your team.

• Collaborate closely with healthcare providers and nursing staff to coordinate services and advocate on behalf of participants.

• Document daily notes comprehensively.

• Facilitate family conferences.

• Provide assistance with community referrals.

• Maintain consistent communication with participants.

• Complete incident reports as necessary.

⛳️ Requirements

• A bachelor's or master's degree in social work from an accredited institution is mandatory.

• A minimum of 2 years of experience in case management and social advocacy is required.

• Experience with the homeless population is preferred.

• Experience in Managed Care is also preferred.

• Bilingual proficiency in English and Spanish is strongly preferred.
