Social Worker I

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee AltaMed Managed Care participants that are assigned to the team.

• Collaborate closely with healthcare providers and nurses to coordinate services and advocate for participants.

• Document daily notes.

• Facilitate family meetings.

• Assist with referrals to community resources.

• Maintain consistent communication with participants.

• Complete reports on incidents.

⛳️ Requirements

• A bachelor's or master's degree in social work from an accredited institution is mandatory.

• At least 2 years of experience in case management and social advocacy is essential.

• Experience working with the homeless population is advantageous.

• Managed Care experience is preferred.

• Bilingual proficiency in English and Spanish is highly desirable.
