Service Coordinator – Nursing Home Transition

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Assist in identifying the unique needs of participants and facilitate their transition from nursing facilities back to the community.

• Support participants in exploring housing options.

• Aid participants in accessing utility service options.

• Collaborate with nursing home personnel and family members of participants as required.

• Verify waiver or income funding availability.

• Comply with state and federal laws/regulations as well as the contractual obligations of contracted Health Plans/Managed Care Organizations.

• Travel within the designated service area.

⛳️ Requirements

• High School Diploma or GED.

• Proven ability to communicate effectively and deliver outstanding customer service.

• Demonstrated skill in organizing multiple tasks efficiently and within deadlines.

• Willingness to travel throughout the assigned area up to 50% of the week.

• Proficient in Microsoft Office Suite, Outlook, and Adobe PDF.

• Valid driver's license with the capacity to travel for business using a personal vehicle.

• Ability to obtain ACT 33/34 and FBI clearances as required.

• Capability to work remotely from home with a reliable internet connection.

• Associate's degree or higher in social work, sociology, social welfare, psychology, gerontology, or a related behavioral science is preferred.

• Three (3) years of experience in a social service or healthcare environment is preferred.

• Familiarity with local housing markets and relationship-building with Providers is preferred.

• Previous experience in Nursing Home Transition services is preferred.

• Bilingual or multilingual skills are preferred.
