Senior VP – Population Health

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Transform the enterprise population health strategy into quantifiable enhancements in clinical documentation, total care costs, quality, and patient outcomes.

• Define and uphold the enterprise population health strategy, roadmap, and key priorities.

• Build and lead a high-performing population health team.

• Direct strategies for precise diagnosis, documentation, and management of chronic conditions, including compliant coding practices.

• Collaborate with Medical Leadership, Market Operations, and Central Operations to enhance Stars, HEDIS scores, preventive care, chronic disease outcomes, and overall patient experience.

• Manage the clinical program portfolio, encompassing prioritization, design, governance, and performance management.

• Identify cost drivers and formulate interventions for high-risk populations, preventable acute utilization, care transitions, medication-related risks, and chronic disease management.

• Establish performance metrics and accountability, track outcomes, and spearhead corrective actions.

• Oversee strategic vendors and external partnerships.

• Represent the organization in interactions with health plans, ACO partners, vendors, and other stakeholders.

• Collaborate on the planning of continuing medical education sessions.

• Engage in Quarterly Operating Reviews and preparation and presentations for Board of Directors meetings.

• Execute other job-related tasks as assigned.

⛳️ Requirements

• Bachelor’s Degree.

• Over 5 years of experience in a senior healthcare leadership role.

• Proficiency in HCC coding and documentation.

• Knowledge of E/M coding and documentation.

• Familiarity with prospective chart review programming.

• Experience in clinical education.

• Extensive knowledge of quality program development and oversight.

• Willingness to travel intermittently to meet business needs.

• Master’s Degree is preferred.
