This is a fully remote position, open to applicants in Florida.
• Assess the appropriateness of pharmacotherapy for high-risk patients with chronic conditions such as diabetes, hypertension, heart failure, and COPD.
• Implement clinical protocols authorized by the organization and state regulatory bodies.
• Enhance drug coverage benefits, formulary options, affordable alternatives, and home delivery pharmacy services.
• Collaborate with physicians, nurses, and other healthcare professionals to improve patient care.
• Engage in multidisciplinary high-risk rounds and modify treatment plans accordingly.
• Advise patients and convey complex medical information to both patients and healthcare providers.
• Instruct patients on disease management, medication usage, and self-care techniques.
• Participate in quality improvement initiatives focused on medication safety and effectiveness.
• Conduct post-discharge medication reconciliation for high-risk transition-of-care patients.
• Record clinical activities in the clinic EMR and other pharmacy or clinical management systems.
• Support CMS Stars and HEDIS quality metrics.
• Keep a record of clinical interventions and patient care activities.
• Provide in-service education and training for healthcare staff.
• Create and disseminate educational materials related to pharmacotherapy.
• Remain informed about advancements in pharmacotherapy and current clinical guidelines.
• Advise healthcare providers on complex medication management challenges.
• Offer support within the pharmacist’s scope of practice to further organizational objectives.
• Execute targeted initiatives as directed by Pharmacy leadership in line with best practices and regulatory standards.
• Report to the Associate Director Integrated Clinic Pharmacist.
• Active and unrestricted pharmacist license in the state of Florida.
• Willingness to obtain licensure in additional states for market support.
• Pharm.D. or another advanced pharmacy degree.
• At least five (5) years of experience as a practicing pharmacist.
• Experience in a collaborative team environment, supporting shared goals.
• Proficiency in MS Office applications, including Word, Excel, PowerPoint, Teams, and Outlook.
• Capability to troubleshoot and address basic technical needs in a remote setting.
• Must live within a 50-mile radius of Englewood Park, West Oakland, West Orange, or Plant City.
• Availability Monday–Friday, from 8:00 a.m. to 5:00 p.m. Eastern Time.
• Minimum home internet speed of 25 Mbps download and 10 Mbps upload.
• Ability to work from a designated space without interruptions to safeguard PHI/HIPAA information.
• Clinical residency in ambulatory care or a related field is preferred.
• Board certification in geriatric pharmacy is preferred.
• Experience in an ambulatory care/primary care setting is preferred.
• Familiarity with EMR systems is preferred.
• Experience in managing medication therapy for patients with chronic conditions is preferred.
• Multilingual capability, preferably in English and Spanish, is preferred.
