Pharmacist – Anticoagulation

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver clinical care to a home-based patient demographic through a centralized anticoagulation program.

• Implement evidence-based medication-use strategies to enhance patient and population health outcomes.

• Examine patient records to identify and address potential drug therapy issues.

• Optimize, recommend, and prescribe or modify medications in accordance with collaborative practice agreements.

• Educate patients and providers on anticoagulation therapy.

• Ensure consistent patient follow-up and monitoring.

• Assess the appropriateness and cost-effectiveness of drug therapy.

• Communicate effectively with patients, providers, staff, service teammates, and pharmacies.

• Enhance anticoagulation workflows and processes.

• Adhere to anticoagulation guidelines, policies, and procedures.

• Accurately document in the EMR.

• Assist in developing reports and tracking service metrics.

• Report adverse drug events and engage in medication quality-improvement initiatives.

• Train practitioners and staff on evidence-based medication practices.

• Support clinical, medical, and operations teams with various ad hoc projects.


⛳️ Requirements

• Doctor of Pharmacy (PharmD) degree.

• Current and unrestricted pharmacy license.

• Minimum of 2 years of experience in clinical pharmacy, including at least 1 year in a role related to anticoagulation services.

• Strong proficiency in Visio, SharePoint, MS Teams, and Office Suite, including Excel and Access.

• Exceptional written and verbal communication skills.

• Skilled in reading, writing, mathematics, multi-tasking, and thriving in a high-pressure environment.

• Ability to work independently with limited supervision.

• Strong interpersonal, analytical, and problem-solving abilities.

• Preferred: Completion of advanced training (PGY1/PGY2).

• Preferred: Certification for anticoagulation (CACP).

• Preferred: Board Certification (BCPS, BCACP, BCGP, etc.).

• Preferred: Familiarity with value-based care initiatives, ACO, MSSP, Stars, and/or HEDIS measures.

• Preferred: Experience with an EMR system.

• Preferred: Background in a healthcare clinic or primary care setting.
