This is a fully remote position, open to applicants in Florida, +1 more state.
• Assess the appropriateness of pharmacotherapy for patients at high risk with chronic illnesses such as diabetes, hypertension, heart failure, and COPD.
• Implement clinical protocols as permitted by the organization and state regulatory bodies.
• Enhance drug coverage benefits, which includes preferred formulary products, cost-effective alternatives, and home delivery pharmacy services.
• Collaborate with physicians, nurses, and other healthcare professionals to enhance patient care.
• Take part in multidisciplinary rounds for high-risk patients and modify treatment plans as necessary.
• Provide counseling to patients and convey complex medical information to both patients and healthcare providers.
• Educate patients on various disease states, proper medication usage, and self-management techniques.
• Participate in initiatives aimed at improving quality, concentrating on medication safety and effectiveness.
• Conduct post-discharge medication reconciliation for patients transitioning from care in high-risk situations.
• Record clinical activities in the clinic's EMR and other pharmacy or clinical management platforms.
• Contribute to CMS Stars and HEDIS quality measures.
• Maintain thorough documentation of clinical interventions and patient care activities.
• Provide in-service education and training to healthcare staff.
• Create and disseminate educational materials on pharmacotherapy.
• Keep up-to-date with advancements in pharmacotherapy and clinical guidelines.
• Advise healthcare providers on complex medication management challenges.
• Offer support within the pharmacist's designated scope of practice to help achieve organizational objectives.
• Execute targeted initiatives and priorities as instructed by Pharmacy leadership.
• Report to the Associate Director Integrated Clinic Pharmacist and integrate pharmacy services into the physician-practice care team.
• Must hold an active and unrestricted pharmacist license in Florida.
• Willingness to obtain licensure in additional states for market support is required.
• Pharm.D. or another advanced pharmacy degree is necessary.
• At least five (5) years of experience as a practicing pharmacist is required.
• Proven experience working collaboratively in a team environment and supporting shared goals.
• Proficient in using MS Office applications including Word, Excel, PowerPoint, Teams, and Outlook.
• Capability to troubleshoot and address basic technical needs in a remote setting.
• Multilingual candidates are preferred, especially those fluent in English and Spanish; must pass both language assessments.
• Clinical residency in ambulatory care or a related field is preferred.
• Board certification in geriatric pharmacy is preferred.
• Preferred experience in an ambulatory care or primary practice environment.
• Familiarity with electronic medical record (EMR) systems is preferred.
• Experience managing drug therapy for patients with chronic conditions is preferred.
• Ability to develop and implement innovative solutions to improve efficiency and patient outcomes.
• Candidates must reside within a 50-mile radius of Corpus Christi or El Paso.
• Availability is required Monday through Friday from 8:00 a.m. to 5:00 p.m. Eastern Time.
• Willingness to travel approximately 5% of the time is necessary.
