Senior Integrated Clinic Pharmacist

This is a fully remote position, open to applicants in Texas.

📋 Description

• Assess the appropriateness of pharmacotherapy for high-risk patients with chronic illnesses such as diabetes, hypertension, heart failure, and COPD.

• Implement clinical protocols in accordance with organizational and state regulatory standards.

• Maximize drug coverage advantages by utilizing preferred formulary products, cost-effective alternatives, and home delivery pharmacy services.

• Work collaboratively with physicians, nurses, and other healthcare professionals to enhance patient care.

• Engage in multidisciplinary rounds focused on high-risk patients and modify treatment plans as necessary.

• Provide counseling to patients and convey intricate medical information to both patients and healthcare providers.

• Offer education on disease management, medication usage, and self-care strategies.

• Participate in initiatives aimed at improving medication safety and effectiveness.

• Conduct medication reconciliation for high-risk patients transitioning from hospital discharge.

• Record clinical activities in the clinic's electronic medical record (EMR) and other pharmacy or clinical management systems.

• Contribute to CMS Stars and HEDIS quality metrics.

• Keep a record of clinical interventions and patient care activities.

• Deliver in-service education and training to healthcare personnel.

• Create and disseminate educational materials related to pharmacotherapy.

• Remain updated on advancements in pharmacotherapy and clinical guidelines.

• Advise healthcare providers on intricate medication management challenges.

• Offer support within the pharmacist's scope of practice to further organizational objectives.

• Execute targeted initiatives as directed by Pharmacy leadership in alignment with best practices and regulatory requirements.

⛳️ Requirements

• Possess an active and unrestricted pharmacist license in the state of Florida.

• Willing to obtain licensure in additional states for market support.

• Hold a Pharm.D. or another advanced pharmacy degree.

• Have a minimum of five (5) years of experience as a practicing pharmacist.

• Experience in a team-oriented environment and supporting shared goals.

• Proficient in MS Office applications including Word, Excel, PowerPoint, Teams, and Outlook.

• Capable of troubleshooting and addressing basic technical issues in a remote setting.

• Multilingual, preferably in English and Spanish; must successfully pass both language assessments.

• Clinical residency in ambulatory care or a related field is preferred.

• Board certification in geriatric pharmacy is preferred.

• Experience in an ambulatory care or primary practice setting is preferred.

• Familiarity with electronic medical record (EMR) systems is preferred.

• Experience managing medication therapies for patients with chronic conditions is preferred.

• Must reside within a 50-mile radius of Corpus Christi or El Paso.

• Ability to work Monday through Friday from 8:00 a.m. to 5:00 p.m. Eastern Time.

• Home internet connection with a minimum of 25 Mbps download and 10 Mbps upload speed.

• Capability to work from a designated area free from ongoing interruptions to safeguard member PHI/HIPAA information.

• Willingness to travel approximately 5% for market meetings and as needed.
