Part-Time Clinical Staff Pharmacist – Antimicrobial Stewardship

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver antimicrobial stewardship services by conducting prospective assessments of antimicrobial therapies.

• Recognize opportunities to enhance antimicrobial selection, dosing, administration route, duration, and monitoring.

• Assess antimicrobial therapy utilizing patient-specific clinical details, microbiology and susceptibility data, laboratory results, organ function, medication history, and allergies.

• Identify chances for antimicrobial de-escalation, escalation, discontinuation, IV-to-oral conversion, dose refinement, and duration adjustments.

• Evaluate the appropriateness of therapy based on infection source, culture and susceptibility results, patient-specific factors, institutional protocols, and contemporary evidence.

• Formulate and relay antimicrobial stewardship recommendations to prescribers, pharmacists, and other healthcare team members.

• Collaborate with physicians, pharmacists, nurses, infection prevention specialists, microbiology staff, and interdisciplinary teams.

• Implement infectious diseases guidelines, clinical literature, local resistance patterns, and client-specific protocols.

• Monitor antimicrobial therapies for effectiveness, safety, interactions, adverse effects, therapeutic drug monitoring, and necessary dose adjustments.

• Record clinical reviews, interventions, recommendations, communications with providers, acceptance, and outcomes.

• Contribute to quality improvement, data collection, clinical education, protocol development, and other ASP initiatives.

• Educate pharmacists, pharmacy technicians, healthcare professionals, and team members regarding antimicrobial stewardship.

• Offer additional clinical pharmacy services such as medication therapy management (MTM), medication reconciliation, transitions of care, pharmacogenomics consultations, medication monitoring, adherence, and quality initiatives.

• Uphold clinical, quality, productivity, and documentation standards.

• Adhere to clinical protocols, client requirements, policies, procedures, standards, and regulations.

• Engage in training, quality improvement, and the development and implementation of new clinical programs.

• Perform other duties as assigned by the Director of Clinical Services, Manager, and Supervisor.

⛳️ Requirements

• Graduate from an accredited college of pharmacy with a Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree.

• Hold an active pharmacist license in good standing.

• Ability to obtain and maintain licensure in CO, OR, MT, IL, and CA as necessary.

• Clinical pharmacy experience with proven knowledge of infectious diseases, antimicrobial pharmacotherapy, and principles of antimicrobial stewardship.

• Completion of a recognized antimicrobial stewardship certificate or training program, a PGY2 Infectious Diseases Pharmacy Residency, or over 5 years of professional experience in antimicrobial stewardship and/or infectious diseases pharmacy services.

• PGY1 pharmacy residency training, board certification, or additional advanced clinical pharmacy training is preferred.

• Extensive knowledge of antimicrobial pharmacotherapy, microbiology, antimicrobial resistance, pharmacokinetics/pharmacodynamics, renal dosing, therapeutic drug monitoring, and evidence-based guidelines for treating infectious diseases.

• Capability to independently assess complex patient-specific clinical information and create evidence-based medication therapy recommendations.

• Strong clinical judgment, critical-thinking, and problem-solving abilities.

• Excellent verbal and written communication skills for conveying clinical recommendations to healthcare professionals.

• Robust organizational and time-management skills.

• Ability to work independently in a remote setting while engaging with a multidisciplinary clinical team.

• Capacity to learn and operate across multiple clinical programs and adapt to changing requirements.

• Proficiency with computer systems, electronic health records, clinical documentation platforms, pharmacy software, clinical drug information resources, and standard productivity applications.

• Willingness to participate in weekend rotations every 4 weekends and at least 3 holidays each year.

• Home internet access via cable broadband or fiber optic service with a minimum of 10 Mbps download and 5 Mbps upload speed.
