Part-Time Clinical Staff Pharmacist – Antimicrobial Stewardship

This is a fully remote position, open to applicants in California, +4 more states.

📋 Description

• Deliver antimicrobial stewardship services through the proactive assessment of antimicrobial treatment.

• Pinpoint opportunities for enhancing antimicrobial selection, dosage, administration route, duration, monitoring, de-escalation, escalation, discontinuation, IV-to-oral conversion, and optimization of dose or duration.

• Assess antimicrobial therapy utilizing patient-specific clinical data, microbiology and susceptibility findings, laboratory results, organ function, medication history, allergies, guidelines, and evidence-based recommendations.

• Formulate and convey antimicrobial stewardship suggestions to prescribers, pharmacists, and members of the healthcare team.

• Collaborate with physicians, pharmacists, nursing staff, infection prevention specialists, microbiology personnel, and other interdisciplinary team members.

• Supervise antimicrobial therapy for effectiveness, safety, drug interactions, adverse effects, therapeutic drug monitoring requirements, and dose adjustments.

• Record clinical reviews, interventions, suggestions, communications with providers, acceptance of recommendations, and outcomes.

• Assist quality improvement initiatives, data collection, clinical education, protocol development, and additional program activities.

• Provide educational resources and clinical support concerning antimicrobial stewardship principles and practices.

• Offer supplementary clinical pharmacy services such as medication therapy management, medication reconciliation, transitions of care, pharmacogenomic consultations, clinical medication monitoring, adherence and quality initiatives, and other designated programs.

• Uphold clinical, quality, productivity, and documentation standards.

• Adhere to relevant protocols, client requirements, policies, procedures, standards, and regulations.

• Engage in training, quality improvement activities, and the development and implementation of new clinical programs.

• Perform other responsibilities as assigned by the Director of Clinical Services, Manager, and Supervisor.

⛳️ Requirements

• Graduate from an accredited college of pharmacy holding a Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree.

• Active pharmacist license in good standing.

• Capability to obtain and maintain licensure in multiple states (CO, OR, MT, IL, CA) as necessary.

• Clinical pharmacy experience showcasing knowledge of infectious diseases, antimicrobial pharmacotherapy, and antimicrobial stewardship principles.

• Completion of a recognized antimicrobial stewardship certificate or training program, a PGY2 Infectious Diseases Pharmacy Residency, or over 5 years of professional experience in antimicrobial stewardship and/or infectious diseases pharmacy services is required.

• PGY1 pharmacy residency training, board certification, or other advanced clinical pharmacy training is preferred.

• Strong understanding of antimicrobial pharmacotherapy, microbiology, antimicrobial resistance, pharmacokinetics/pharmacodynamics, renal dosing, therapeutic drug monitoring, and evidence-based infectious diseases treatment guidelines.

• Ability to independently analyze complex patient-specific clinical information and generate evidence-based medication therapy recommendations.

• Strong clinical judgment, critical-thinking, and problem-solving capabilities.

• Excellent verbal and written communication skills.

• Strong organizational and time-management abilities.

• Capacity to work autonomously in a remote setting while effectively collaborating with a multidisciplinary clinical team.

• Ability to learn and operate across various clinical programs and adapt to changing client and organizational needs.

• Proficient in using computer systems, electronic health records, clinical documentation platforms, pharmacy software, clinical drug information resources, and standard productivity applications.

• Willingness to participate in a weekend rotation once every four weekends and at least three holidays per year.

• Home internet connection via cable broadband or fiber optic service with minimum speeds of 10Mbps download and 5Mbps upload.
