This is a fully remote position, open to applicants in Florida.
• Assess the appropriateness of pharmacotherapy for high-risk patients with chronic illnesses utilizing evidence-based medicine and clinical expertise.
• Implement clinical protocols in alignment with organizational and state regulatory standards.
• Enhance medication coverage benefits through preferred formulary products, cost-effective alternatives, and home delivery pharmacy services.
• Collaborate effectively with physicians, nurses, and other healthcare professionals to improve patient care.
• Engage in multidisciplinary high-risk rounds and modify treatment plans as necessary.
• Provide counseling to patients and convey intricate medical information to both patients and healthcare providers.
• Offer education on disease states concerning medication use and self-management practices.
• Participate in quality improvement initiatives concentrated on medication safety and effectiveness.
• Conduct post-discharge medication reconciliation for patients transitioning from care.
• Document clinical activities in the clinic's EMR and other pharmacy or clinical management systems.
• Support CMS Stars and HEDIS quality measures effectively.
• Ensure precise documentation of clinical interventions and patient care actions.
• Facilitate in-service education and training sessions for healthcare personnel.
• Create and disseminate educational materials related to pharmacotherapy.
• Keep up-to-date with advancements in pharmacotherapy and clinical guidelines.
• Advise healthcare providers on complex medication management challenges.
• Address requests from the Primary Care Organization within the pharmacist’s professional scope.
• Execute targeted initiatives and priorities as directed by pharmacy leadership in accordance with best practices and regulatory requirements.
• Possession of an active and unrestricted pharmacist license in the state of Florida.
• Openness to obtaining licensure in additional states for market support.
• Pharm.D. or an equivalent advanced pharmacy degree.
• A minimum of five (5) years of experience as a practicing pharmacist.
• Experience collaborating in a team environment to achieve shared goals.
• Proficiency in MS Office applications, including Word, Excel, PowerPoint, Teams, and Outlook.
• Capability to troubleshoot and resolve basic technical issues within a remote setting.
• Clinical residency in ambulatory care or a similar field (preferred).
• Board certification in geriatric pharmacy (preferred).
• Experience in an ambulatory care or primary practice environment (preferred).
• Familiarity with electronic medical record (EMR) systems (preferred).
• Experience managing drug therapies for patients with chronic conditions (preferred).
• Aptitude for identifying and implementing innovative solutions to enhance efficiencies and outcomes (preferred).
• Multilingual abilities, preferably in English and Spanish (preferred).
• Must live within a 50-mile radius of Okeechobee or Lake Worth, Florida.
• Availability from Monday to Friday, 8:00 a.m. to 5:00 p.m. Eastern Time.
• Willingness to travel approximately 5%, typically once a quarter for market meetings.
• Access to home internet with a minimum download speed of 25 Mbps and upload speed of 10 Mbps.
