Clinical Pharmacist

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Take charge of patients’ pharmacotherapy outcomes under the supervision of the Director or Manager.

• Act as a liaison for patients, families, pharmacists, clients, and healthcare professionals regarding medication-related issues and pharmacotherapy requirements.

• Clarify care plan choices and provide information about medication usage, side effects, and contraindications.

• Record patient care decisions, actions, coding, and billing using specialized software.

• Develop, execute, and uphold procedures for evaluating, compounding, packaging, labeling, and disposing of medications.

• Supervise Pharmacy Technicians performing medication reconciliation, physician order entry, inventory management, medication storage, security, labeling, and packaging.

• Review, prepare, compound, and give final approval for prescription orders and medications dispensed.

• Confirm the accuracy of drug, dosage, directions, quantity, days’ supply, refills, packaging, labeling, ancillary warnings, patient profile, and dispensing.

• Ensure adherence to compliant billing and recordkeeping for dispensed and shipped medications.

• Deliver pharmaceutical care, evaluate medication requests and refills, and suggest interventions to healthcare providers.

• Maintain compliance with state and federal laws, regulations, HIPAA, Fraud, Waste, and Abuse requirements, and internal policies.

• Identify, report, and address quality assurance issues, HIPAA violations, Fraud, Waste, and Abuse, and other infractions.

⛳️ Requirements

• Bachelor’s degree in Pharmacy or PharmD preferred.

• Current, active license in the state of New Jersey.

• Must be registered or eligible for registration with the appropriate State Board of Pharmacy.

• At least 2 years of experience in pharmacy distribution and packaging.

• Experience with geriatrics and/or chronic care management.

• Proficient in MS Office Suite.

• Strong technical and computer skills for proprietary and pharmacy software.

• Understanding of state and federal laws and regulations, including HIPAA and Fraud, Waste, and Abuse.

• Exceptional verbal and written communication abilities.

• Skills in problem-solving, conflict resolution, and project management.

• Strong analytical capabilities and clinical judgment.

• Ability to identify and resolve medication-related issues.

• Capability to assess patients using verbal and/or visual cues.

• Detail-oriented and organized, able to perform under pressure, multitask, and meet deadlines.

• Availability to work an average of 40 hours per week and outside normal business hours on an ad-hoc basis.

• Willingness to participate in a rotating holiday schedule.
