Senior Care Manager, RN

This is a fully remote position, open to applicants in New York.

📋 Description

• Evaluate, design, and execute intricate care management initiatives

• Create and consistently review individualized care/service plans

• Collaborate with healthcare providers, specialists, community resources, members, families, caregivers, and care teams

• Oversee and coordinate member care and services

• Track member status, changes in condition, progress, and goals outlined in care plans

• Identify issues, obstacles, and interventions for escalated cases

• Analyze member data, referrals, and intake assessments

• Conduct outreach via phone, digital platforms, home visits, and/or on-site visits

• Gather, document, and uphold member and care management information to ensure regulatory compliance

• Educate members and caregivers about disease processes, care gaps, treatment guidelines, care options, referrals, and benefits

• Collaborate with leadership to enhance care quality and cost efficiency

• Mentor, coach, and assist new clinical hires during their onboarding process

• Complete competency assessments and evaluate preceptee readiness

• Work alongside People Leaders and cross-functional teams

• Perform additional assigned tasks and adhere to policies and standards

⛳️ Requirements

• Must possess a degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

• 4–6 years of relevant experience required

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory

• NY RN State Licensure is a must

• Ability to work Monday–Friday, 8am–5pm EST
