This is a fully remote position, open to applicants in Louisiana.
• Evaluate and assess the needs and requirements of members to achieve or maintain optimal wellness.
• Direct members and their families towards suitable care and wellbeing resources.
• Oversee members' physical, environmental, and psychosocial health concerns.
• Identify and address obstacles to effective care.
• Involve members and families in recovery and wellness programs through a holistic, member-centric approach.
• Perform both telephonic and in-person assessments.
• Track and assess member progress.
• Create personalized care plans utilizing clinical judgment and changes in health status.
• Work collaboratively with providers and community services to guarantee quality and cost-effective outcomes.
• Coordinate physical health, social determinants of health, and additional benefits.
• Facilitate communication among the care team, including primary care providers and during care transitions.
• Provide health and wellness resources to families.
• Complete incident reports as necessary.
• Take part in health promotion and community events.
• Execute care management activities in compliance with contractual obligations, established guidelines, and directives from the Louisiana Department of Health.
• Conduct face-to-face interactions with members, families, and providers.
• Travel within the designated Louisiana region and participate in community engagement activities and team meetings.
• Must live in Region 1, 3, or 9 of Southern Louisiana, which includes Assumption, Jefferson, Lafourche, Livingston, Orleans, Plaquemines, St. Bernard, St. Charles, St. Helena, St. James, St. John, St. Mary, St. Tammany, Tangipahoa, Terrebonne, or Washington parishes.
• Current, unrestricted Louisiana Registered Nurse (RN) license.
• A minimum of two years of clinical experience in maternal-child health, Labor & Delivery (L&D), and/or pediatrics.
• Intermediate to advanced proficiency in computer skills.
• Experience using Microsoft Word, Outlook, and Excel.
• Capability to utilize electronic information applications/software, including electronic medical records.
• Excellent communication and interpersonal abilities.
• Capacity to work independently with minimal supervision and demonstrate sound judgment.
• Case Management Certification (CCM) or a commitment to obtain it within two years of employment.
• Knowledge of current state Maternal/Child Mortality Morbidity.
• Basic understanding of breastfeeding.
• Experience with Medicare, Medicaid, and dual-eligible populations.
• Field Case Management experience.
• Experience in Managed Care Health Plans.
• Familiarity with community health and social service agencies and resources.
• Background in health promotion, coaching, and wellness.
• Experience with Motivational Interviewing.
• Must meet home internet speed requirements of at least 25 Mbps download and 10 Mbps upload.
• Must work from a designated area to safeguard member PHI/HIPAA information.
