This is a fully remote position, open to applicants in Louisiana.
• Evaluate and assess the needs and requirements of members to support or sustain optimal wellness.
• Direct members and their families towards suitable care and wellness resources.
• Oversee members' physical, environmental, and psychosocial health concerns.
• Identify and address obstacles to effective care delivery.
• Involve members and their families in recovery and wellness initiatives through a holistic, member-centered approach.
• Perform assessments via phone and in-person.
• Track and assess member progress.
• Create personalized care plans utilizing clinical judgment and changes in health status.
• Partner with providers and community organizations to ensure quality, cost-effective outcomes.
• Coordinate physical health, social determinants of health, and additional benefits.
• Enhance communication among the care team, including Primary Care Providers (PCPs) and during care transitions.
• Offer families resources related to health and wellness.
• Report incidents as necessary.
• Engage in health promotion and community events.
• Execute care management activities in accordance with contractual obligations, established protocols, and directives from the Louisiana Department of Health.
• Conduct face-to-face interactions with members, their families, and providers.
• Travel up to 50% within the designated Louisiana region.
• Must reside in Region 6, 7, or 8 of Northern Louisiana, including the specified eligible parishes.
• Active Registered Nurse (RN) license in Louisiana without any restrictions.
• A minimum of two years of clinical experience in maternal-child health, Labor & Delivery (L&D), and/or pediatrics.
• Proficient to advanced computer skills.
• Familiarity with Microsoft Word, Outlook, and Excel.
• Ability to utilize electronic information applications/software, including electronic medical records.
• Excellent communication and interpersonal abilities.
• Capability to work independently with minimal oversight and demonstrate sound judgment.
• Case Management Certification (CCM) or a commitment to obtain within two years of employment.
• Awareness of current state Maternal/Child Mortality Morbidity.
• Basic knowledge of breastfeeding.
• Understanding or experience with Medicare, Medicaid, and dual-eligible populations.
• Experience in Field Case Management.
• Background in Managed Care Health Plans.
• Familiarity with community health and social service agencies along with community resources.
• Experience in health promotion, coaching, and wellness.
• Experience in Motivational Interviewing.
• Must satisfy work-at-home internet requirements: minimum 25 Mbps download and 10 Mbps upload.
• Must work from a designated area without ongoing interruptions to safeguard member PHI/HIPAA information.
• Current or former Louisiana Health Department personnel engaged directly with LDH's Medicaid Division must have a separation of two or more years from LDH for consideration.
