This is a fully remote position, open to applicants in California, +2 more states.
• Coordinate and oversee the delivery of quality, cost-effective care for patients with ESRD and CKD.
• Reduce fragmentation within the healthcare delivery system.
• Operate independently while collaborating with the healthcare team.
• Manage care coordination among patients, families, physicians, providers, care teams, community resources, and the Regional Operations Manager.
• Provide support to ESKD and CKD patients throughout California, Oregon, Nevada, and Washington as needed.
• Address patient needs and nephrology partner availability from Monday to Friday.
• An ADN degree from an accredited nursing school is required; alternatively, a three-year diploma from an accredited diploma program may substitute for the nursing degree.
• A BSN is preferred.
• A current Registered Nurse (RN) license in the state of practice is required; must possess or obtain licensure in California, Oregon, Nevada, or Washington as applicable.
• Continuing education credits must be maintained as mandated by the state of practice.
• A minimum of five years’ experience in clinical nursing is strongly preferred.
• A minimum of three years’ experience in renal nursing is preferred.
• Case management experience is required or alternative experience of 3+ years in assessing patient needs, developing and managing individualized care plans, coordinating interdisciplinary services, and supporting complex healthcare transitions.
• Certification as a Nephrology Nurse (CNN) or Certified Case Manager (CCM) is preferred.
• Familiarity with data and management of clinical, financial, and patient satisfaction outcomes.
• Proven experience and effectiveness in a change agent role.
• Knowledge of Continuous Quality Improvement (CQI) techniques.
• Prior experience in healthcare performance coaching is required.
• Current CPR certification is required.
• Capability to adapt to new/electronic processes and increased productivity expectations for Capella implementation.
• A current driver’s license in the state of employment, a positive driving record, and the ability to meet insurance coverage requirements is necessary.
• Basic computer skills with proficiency in MS Word and Outlook.
• Functional proficiency with DaVita clinical software programs, including Capella, must be achieved within 90 days of employment.
