RN Care Manager – Lead

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Deliver lead care management services through the Family Care Partnership program.

• Guide and mentor FCP nurses and care coaches.

• Organize field shadowing and onboarding applications for new care managers and nurses during their orientation.

• Communicate employee engagement and progress in mentoring activities to management.

• Educate and coach staff on care management, RN responsibilities, interdisciplinary collaboration, and compliance with contractual obligations.

• Provide ongoing training on the Family Care model and additional best practices.

• Identify, coordinate, implement, and assess external training programs.

• Perform nursing assessments during initial comprehensive assessments and follow-up evaluations.

• Utilize Resource Allocation Decision-making methodology.

• Coordinate and oversee services and resources that support member outcomes.

• Conduct RN reassessments, update care plans, and supervise member health and safety.

• Assist members and families in accessing medical, social, rehabilitation, vocational, educational, and other essential services.

• Support members or their representatives with complaints, grievances, and advocacy services.

• Collaborate with the Interdisciplinary Team to address complex medical, psychiatric, behavioral health, and social needs.

⛳️ Requirements

• Valid Registered Nurse license in the State of Wisconsin, in good standing.

• Minimum of 2 years of experience in Managed Care.

• Must live within a reasonable commuting distance from the Family Care Partnership service area in Southeast Wisconsin.

• Willingness to travel approximately 20% within Kenosha, Racine, Milwaukee, Waukesha, Walworth, Washington, and Sheboygan.

• Possession of a valid state driver's license.

• Proof of personal vehicle liability insurance with minimum limits of 100/300/100.

• Patient-facing role necessitating TB screening.

• Minimum home internet speed of 25 Mbps for downloads and 10 Mbps for uploads.

• Ability to work from a dedicated space that safeguards member PHI/HIPAA information.

• Preferred: Bilingual proficiency in English and Spanish.

• Preferred: Experience in training or mentoring.

• Preferred: Familiarity with community health and social service agencies and resources.
