RN Care Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Evaluate the health and safety requirements of members to create detailed Member Care Plans.

• Perform regular health evaluations.

• Collaborate with acute and primary care providers.

• Educate members about disease processes, risks, nutrition, exercise, and lifestyle changes.

• Maintain documentation in member files in accordance with required DHS contract deadlines.

• Engage in monthly phone communications with members.

• Travel to members' locations for in-person consultations.

• Conduct record reviews for reassigned members and identify areas needing remediation.

• Assist with record review and remediation tasks.

• Finalize remediation documentation.

• Notify newly assigned RNs and Care Coach partners of any missing or required follow-up actions.

• Delegate tasks within the D365 system.

• Accept additional complex member cases.

• Offer extra assistance with new member assignments.

• Travel approximately 40% of the time to meet with members.

⛳️ Requirements

• Associate degree in nursing.

• Licensed Registered Nurse in Wisconsin, maintaining good standing.

• Preferred 3 years of experience in Care Management.

• Ideally, reside within a 45-minute radius of the assigned coverage area.

• Previous experience in Care Management is preferred.

• Valid state driver's license required.

• Must provide proof of personal vehicle liability insurance with minimum limits of 100/300/100.

• Pass TB screening upon selection.

• Must have home internet with at least 25 Mbps download and 10 Mbps upload speeds.

• Dedicated workspace free from interruptions to ensure the protection of member PHI/HIPAA information.

• Availability to work Monday through Friday, from 8:00 am to 4:30 pm CST.
