RN Care Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Evaluate the health and safety requirements of members to create thorough Member Care Plans.

• Perform regular health evaluations.

• Collaborate with both acute and primary care providers.

• Provide education to members regarding disease processes, associated risks, nutrition, exercise, and lifestyle changes.

• Maintain documentation in member files within the specified DHS contract deadlines.

• Engage in monthly phone outreach to members and travel for in-person visits in their environments.

• Conduct record reviews for reassigned members and pinpoint areas needing remediation.

• Assist with record review and remediation efforts.

• Finalize documentation for remediation activities.

• Notify newly assigned RNs and Care Coach partners about any missing or required follow-up actions.

• Delegate tasks within the D365 system.

• Take on additional complex member assignments as needed.

• Aid in new member assignments.

• Work remotely from home while traveling approximately 40% of the time to visit members.

• Report directly to the Manager of Care Coaching.

⛳️ Requirements

• Associate degree in nursing is required.

• Must be a Licensed Registered Nurse in Wisconsin, in good standing.

• A valid state driver’s license is necessary.

• Proof of personal vehicle liability insurance with minimum limits of 100/300/100 is required.

• TB screening is mandated for the patient-facing role if selected.

• A dedicated workspace free from interruptions is essential to safeguard member PHI/HIPAA information.

• Minimum home internet download speed of 25 Mbps and upload speed of 10 Mbps is required.

• Preferred: At least 3 years of experience in Care Management.

• Preferred: Residing within a 45-minute commute or less of the assigned coverage area.

• Previous experience in Care Management is preferred.
