RN Care Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Evaluate the health and safety requirements of members to create thorough Member Care Plans.

• Conduct routine health evaluations.

• Collaborate with acute and primary care providers.

• Provide education to members regarding disease processes, associated risks, nutrition, exercise, and lifestyle changes.

• Maintain documentation in member files within the stipulated DHS contract timelines.

• Engage in monthly member phone outreach and travel for in-person visits.

• Perform record reviews for reassigned members and determine remediation needs.

• Assist with record review and remediation efforts.

• Complete necessary remediation documentation.

• Notify newly assigned RNs and Care Coach partners of any missing or required follow-up tasks.

• Delegate tasks within the D365 system.

• Take on additional complex member cases as needed.

• Aid in the onboarding of new member assignments.

• Travel approximately 40% of the time for member visits.

• Facilitate care coordination, support, and education for members of the Wisconsin Family Care Program.

⛳️ Requirements

• Associate degree in nursing.

• Licensed Registered Nurse in the state of Wisconsin (in good standing).

• Valid driver's license for the state.

• Proof of personal vehicle liability insurance with minimum limits of 100/300/100.

• Ability to work Monday through Friday from 8:00 am to 4:30 pm CST.

• Capacity to work remotely from a designated area without continuous interruptions.

• Minimum internet speed of 25 Mbps download and 10 Mbps upload required at home.

• Patient-facing role necessitating TB screening upon selection.

• Preference for candidates residing within 45 minutes or less of the designated coverage area.

• Preferred 3 years of experience in Care Management.
