RN Care Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Evaluate the health and safety requirements of members to create thorough Member Care Plans.

• Perform regular health evaluations.

• Collaborate with acute and primary care providers.

• Educate members about disease processes, associated risks, nutrition, exercise, and lifestyle changes.

• Document member information within the required DHS contract deadlines.

• Engage in monthly member phone interactions.

• Travel to conduct in-person visits in members' environments.

• Carry out record reviews for reassigned members and identify necessary record corrections.

• Assist the team with record reviewing and remediation efforts.

• Complete documentation of remediation actions.

• Notify newly assigned RNs and Care Coach partners of any missing or required follow-up.

• Allocate tasks within the D365 system.

• Accept additional complex member assignments as needed.

• Provide assistance with new member placements.

• Report to the Manager of Care Coaching.

• Support members enrolled in the Wisconsin Family Care Program through care coordination, assistance, and education.

⛳️ Requirements

• Associate degree in nursing.

• Valid Registered Nurse license in the state of Wisconsin, in good standing.

• Preferred 3 years of experience in Care Management.

• Preferred residence within a 45-minute commute or less from the assigned coverage area.

• Previous experience in Care Management is preferred.

• Valid state driver's license.

• Proof of personal vehicle liability insurance with minimum limits of 100/300/100.

• Must undergo TB screening if selected.

• Home internet connection with a minimum download speed of 25 Mbps and upload speed of 10 Mbps.

• Dedicated workspace free from interruptions to safeguard member PHI/HIPAA information.

• Ability to work Monday through Friday, from 8:00 am to 4:30 pm CST.

• Willingness and capacity to travel approximately 40% of the time.
