This is a fully remote position, open to applicants in Florida, +6 more states.
• Perform comprehensive evaluations of existing medical, psychological, and complex social needs.
• Recognize obstacles to care, such as food insecurity, unstable housing, transportation difficulties, and insurance restrictions.
• Work collaboratively with families and multidisciplinary teams to set personalized care objectives.
• Create and uphold integrated care plans that address medical, psychological, and social requirements.
• Ensure prompt follow-up on referrals, assessments, and treatment recommendations.
• Educate families on how to access local resources and agencies.
• Provide emotional support and guidance in challenging circumstances.
• Develop safety plans for situations involving interpersonal/domestic violence and caregiver health concerns.
• Advocate for necessary resources, eliminate obstacles, and connect families with available support.
• Establish and nurture relationships with community organizations that assist children with medical and behavioral health challenges.
• Stay updated on new programs and resources relevant to the population served.
• Represent Imagine Pediatrics at community events, health fairs, and resource meetings.
• Engage in scheduled interdisciplinary consultations to track patient progress.
• Carry out additional tasks as assigned.
• Master’s degree in social work is mandatory.
• LMSW or equivalent state licensure is required.
• Candidates must hold a license in NY, NJ, NC, GA, FL, TX, MO, TN, or Washington DC.
• 2–3+ years of postgraduate experience in social service case management is essential.
• At least 1 year of experience working with pediatric populations is required.
• Proficiency in motivational interviewing practices and/or techniques is necessary.
• Comfort in working with acute populations and familiarity with school and community resources is highly desirable.
• Experience in providing telehealth services and engaging with families remotely is preferred.
• Bilingual in Spanish is strongly preferred.
• Availability from 8:00 am to 5:00 pm EST, Monday to Friday, with two supervisor-determined days from 10:00 am to 7:00 pm EST.
