Resource Specialist

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Offer resource support to members and link them to local resources based on care/service assessments and plans.

• Assist in evaluating member needs and either develop or contribute to care/service plans.

• Identify and coordinate referrals for care and services.

• Connect members to local resources, gather necessary documentation, and aid in application processes.

• Assess member eligibility and care needs based on evaluations and care/service plans.

• Monitor the delivery of care/services and follow up with members, caregivers, and providers.

• Collaborate with providers, community organizations, stakeholders, and government entities to help members and families access critical services.

• Aid in the creation of educational and training programs for care management staff and providers.

• Support the development of local resource service materials, policies, and procedures.

• Ensure that resource services information is accurately documented and up-to-date.

• Conduct telephonic and in-person outreach to reach hard-to-contact individuals and families.

• Perform other assigned tasks while adhering to policies and standards.

⛳️ Requirements

• Must be authorized to work in the U.S. without requiring employment-based visa sponsorship now or in the future.

• Applicants must reside in North Carolina.

• A Bachelor’s degree along with 2–4 years of relevant experience, or equivalent experience gained through applicable knowledge, duties, scope, and skill.

• Capability to assist members with employment, housing, independent living, justice, foster care, and/or community transition services.

• Proficient in collaborating with care teams, providers, community-based organizations, community stakeholders, and governmental entities.

• Competence in conducting telephonic and in-person outreach.

• Must comply with all policies and standards.
