This is a fully remote position, open to applicants in Canada.
• Identify necessary rehabilitation assessments to evaluate Participants’ potential for improvement and specify rehabilitation needs.
• Organize rehabilitation assessments and services as needed.
• Ensure that services and plans are tailored to individual needs, focused on goals, compliant with program mandates, safe, cost-effective, and of high quality.
• Create comprehensive Rehabilitation Plans in collaboration with Participants, families/caregivers, Rehabilitation Service Professionals, and Veterans Affairs Canada Case Managers.
• Serve as the service integrator for medical, psycho-social, and vocational rehabilitation services.
• Empower Participants to engage in problem-solving through community-based interventions and resources.
• Recognize barriers or risk factors that may lead to less-than-optimal outcomes.
• Proactively design and implement strategies aimed at enhancing outcomes.
• Deliver rehabilitation services to veterans of the Canadian Armed Forces and their families.
• Travel minimally across the region as necessary.
• Bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related field.
• One of the following: a health professional designation; Certified Vocational Rehabilitation Professional (CVRP or CVRP-F); or, in Québec, registration as a Career Counsellor with the Ordre des conseillers et conseillères d'orientation du Québec.
• At least 3 years of experience in providing medical, psycho-social, or vocational rehabilitation services, or a combination of these.
• Experience working with individuals facing mental or physical health challenges that pose barriers in their lives.
• Employment is contingent upon obtaining federal security clearance – Reliability Status.
• Willingness to travel across the region as needed.
• Bilingual proficiency in French and English is preferred.
• Experience collaborating with employers, community resources, and training institutions is preferred.
• Current and former members of the Canadian Armed Forces are encouraged to apply.
